Surrogate Qualification

Thank you for considering becoming a Surrogate with Vias. This first step helps us get to know you, including your pregnancies, your health, and the people who support you. It takes about 15 minutes, and your answers save as you go, so you can take a break whenever you need to.

A real person on our team reads every questionnaire. Apart from the few basic requirements on the first page, no single answer decides anything on its own. If something needs a closer look, we'll talk it through with you. If it's a good fit, we'll set up a time to talk, and then invite you to complete your full application.

Chapter 1 of 6About you

Let's start with the basics

Welcome, and thank you for considering this. These first few questions confirm our basic requirements, so you'll know right away whether our program is a good fit before sharing anything more personal.

Date of birth
MM / DD / YYYY
Have you given birth to at least one child?
Our program works with Surrogates whose status will stay in place through screening, pregnancy, and the legal steps after birth.
When does it expire?

Your answers are private. They're read by the Vias team and shared only with the professionals involved in your screening, such as a fertility clinic, psychologist, or attorney. Privacy policy

It's nice to meet you.

Tell us a little about where you are right now. There are no wrong answers here, whether you've been a Surrogate before or you're just beginning to learn about it.

Have you been a Surrogate before?

Your pregnancies

Every pregnancy story is different, and no one has a perfect history. We aren't looking for one. These details help the fertility clinic understand how your body has handled pregnancy before, so they can keep your health and safety in mind from the very beginning.

Please count every pregnancy, including births, miscarriages, ectopic pregnancies, terminations, and any current pregnancy. Twins count as one pregnancy.

Here's what we have

Pregnancy and delivery complications

Check anything you've experienced in any pregnancy. Many of these are more common than people realize, and each one is looked at thoughtfully and in context, never as an automatic no.

Have you had at least one full-term pregnancy without a major complication?

Where things are today

Just a couple of quick questions to help us understand timing.

Are you currently breastfeeding or pumping?

Reproductive and fertility history

You're almost through this chapter. These last few questions help us understand whether there is anything a fertility clinic may want to review more closely later.

Have you ever been diagnosed with or treated for any of these?
Have you had surgery or another procedure on your uterus, cervix, ovaries, or fallopian tubes?
Not counting C-sections.
Have you had fertility treatment or IVF?
Have you ever had an embryo transfer?

Your health

Thank you for sticking with us. Your best recollection is all we need here, so please don't worry about exact dates. If you move forward, a doctor will go over your records with you.

Height
Your BMI
Add your height and weight
Calculated from the height and weight you enter. BMI is just one part of the clinic's overall health review, and it's something we can talk through together.
Have you ever been diagnosed with any of these?
Are you taking any prescription medications right now?
Include dose if you know it.
Have you had any major surgeries or hospital stays, other than giving birth?
Have you ever had a blood transfusion?

Sexual and cervical health

We understand these questions are personal, and we appreciate your openness. Many infections and cervical changes are common and treatable, and sharing them now simply means there are no surprises later.

Have you ever been diagnosed with a sexually transmitted infection?
Which one(s)?
Do you need any treatment or follow-up now?
Have you ever had an abnormal Pap smear, or needed treatment for cervical changes?
When was your most recent Pap smear?
Month and year. Your best guess is fine.

Emotional wellbeing

How you feel matters just as much as how your body is doing. Pregnancy brings emotional changes too, and we'd like to understand what you've been through and what has helped. Having seen a therapist or taken medication doesn't rule anyone out. Share what you're comfortable with here, and anything else can wait until we talk.

Have you ever been diagnosed with or treated for any of these?
Have you ever been hospitalized or needed emergency care for your mental health, including for thoughts of harming yourself?

If you're having a hard time right now, you can call or text the 988 Lifeline, any time, day or night. It's free and confidential.

Nicotine, alcohol, and other substances

Honest answers help most here. Timing matters more than history, so we ask about recent use separately from the past.

Have you ever used other recreational or non-prescribed drugs?

The people around you

No one should go through this alone. Having people who can help with appointments, pregnancy, and recovery makes a real difference, and we'd love to know who's in your corner.

Does your partner know you're considering surrogacy?
Do you have dependable childcare for appointments, travel, rest, and recovery?
Is there at least one person you could lean on, practically and emotionally, during a surrogacy?
We'll ask for their contact details later, only if you move forward.

Your everyday life

Life is already full. These questions help us picture how appointments would fit around your work and family, so we can plan with you, not around you.

Do you have reliable transportation to medical appointments?
Who would usually get you to appointments?
Check all that apply.

How the process works

In gestational surrogacy, the embryo is created through IVF using the Intended Parents' or a donor's egg. The Surrogate isn't genetically related to the baby she carries.

To prepare for the embryo transfer and support early pregnancy, the fertility clinic prescribes medication, usually including injections, for a few months.

If any of this is new to you, that's completely okay. We'll walk you through every step.

Background

Surrogacy includes formal background checks, so it helps for us to know about anything early. Please answer as openly as you can. If something needs more context, we'll talk it through with you.

Have you applied to be a Surrogate with another agency, clinic, or program?
Have you ever been convicted of a crime, or do you have charges pending?
Has any adult who lives in your home been convicted of a crime, or do they have charges pending?
Have you ever been involved with Child Protective Services or another child-welfare agency?
Include any investigation, even one that was closed. Each situation is looked at individually.
Have your parental rights ever been restricted, suspended, or terminated?
Are you currently in a significant custody dispute?

Insurance and benefits

Insurance and benefits can affect how a surrogacy is set up. Knowing now means we can plan around it together and nothing catches you off guard later.

Do you receive government benefits that extra income could affect?
For example Medicaid, SNAP/EBT, TANF, SSI, or housing assistance.

Just a few details

You're nearly finished. We just need your legal name and address for our records.

In your own words

This is the part we look forward to reading most. None of it is required, but it helps us get to know you as a person, not just a list of answers.

What would you like to talk about when we meet?

Before you send this

Thank you for your time and your honesty. Please read and check each of these.

What happens next

  1. Someone on our team reads your answers personally and reaches out within two business days.
  2. We set up a time to talk, a relaxed conversation of about 45 minutes to an hour to get to know each other and answer your questions.
  3. If it feels like the right fit for both of us, we'll invite you to Phase 2, your full application.

If you entered something by mistake, you can go back and change it.

Your answers are saved.

Take whatever time you need.

And if you'd like to talk with someone first, we're happy to hear from you at (949) 270-3300.

Two women talking at a table

We know how much of yourself you shared here. Thank you for trusting us with it.

A member of our team will read your answers personally and reach out within two business days. Your interview is a conversation, not a test. It takes about 45 minutes to an hour, and it's a chance to get to know each other, talk through the process, and answer any questions you have. If it feels like the right fit for both of us, we'll invite you to complete your full application.

If you'd like, you can choose a time to talk right now.

Choose a time to talk

It opens our calendar in a new tab. Pick whatever time works best for you.

We'll reach out to find a time that works for you.

If a question comes up before then, call us at (949) 270-3300 or reply to your confirmation email. You don't have to wait to hear from us.

We're so glad you're here.
