If you complete our Surrogate qualification questionnaire, we collect the information you provide. This includes health information, such as your pregnancy and medical history, reproductive and sexual health, emotional health, and substance use, as well as background, insurance, and contact details.
How we use your information
We use the information from our inquiry forms to understand which path you are exploring (Intended Parent, Surrogate, or general inquiry), to respond to you, and to reach you using your preferred contact method. We use the information from the Surrogate qualification questionnaire only to understand whether our Surrogate program may be a good fit for you and to prepare for your interview.
Who can see your answers
Your information is reviewed by the Vias team. As part of Surrogate screening, relevant information may be shared with the fertility clinic, psychologist, attorney, or insurance specialist involved in your screening. We never sell your information or use it for advertising.
Saving your progress, reminders, and messages
Once you complete the first page of the Surrogate qualification questionnaire, your answers are saved so you can return and finish later on any device. If you leave before finishing, we may email you a link to continue and up to two reminders. We send text messages only if you agree to receive them, and you can reply STOP at any time to opt out.
How submissions are processed
Information you submit through this website is received and stored by the service providers that host our website and help us manage our records and deliver our emails and text messages. They handle it on our behalf so our team can review and respond to you.
